1. If you are going to the same vet you always go to, you don't need to list the vaccine history- your vet will have that on file, and sometimes, the owners don't even know the exact date.
2. If you are going to a new vet, have a place for your old vet's contact information so the new vet can call, or have them fax records or information that you did not bring with you.
3. You may want to break the check list up into two columns- it can become a little overwhelming when it's just one like ALL THE WAY DOWN! This will also save space on paper too.
