Looking at this from a vet point of view, the form is Great. The only suggestions I have are:

1. If you are going to the same vet you always go to, you don't need to list the vaccine history- your vet will have that on file, and sometimes, the owners don't even know the exact date.

2. If you are going to a new vet, have a place for your old vet's contact information so the new vet can call, or have them fax records or information that you did not bring with you.

3. You may want to break the check list up into two columns- it can become a little overwhelming when it's just one like ALL THE WAY DOWN! This will also save space on paper too.

I used this form when I took Tina to the vet a few months ago for her first visit since her adoption. My vet was impressed and put it in her file!

I think this form is very helpful, the only thing to add is if the cat has any history of seizures or painfull area's to touch or any weird behavior. I think this form is a great idea and very important! thanks for thinking about it.

I think that this is a great form especially if you are travelling with your pet and you need to take your cat to a vet other than your own.

I think you should add "Past and Current Medical Conditions with dates". A lot of times people have to take their cat to the vet and are so concerned and focused on the issue at hand they forget some of this important information.

This form is great. I have taken my older cat, Midnight, to the vet after a cat fight and have been too upset to think. Filling out most of the form before it is needed would be a good idea.

A long time ago I came across a great form to provide to your vet. I used it on the old Cat Fancy board. Too many times we are taking sick cats to the vet and we are upset and we can forget to provide important details to the vet. I would GREATLY APPRECIATE if everyone would provide some feedback so we could turn this form into a really good form for our Cat Channel members to use when they take sick animals to the vets.
