The only suggestions I have are:&lt;/P&gt;&lt;P&gt;1. If you are going to the same vet you always go to, you don't need to list the vaccine history- your vet will have that on file, and sometimes, the owners don't even know the exact date.&lt;/P&gt;&lt;P&gt;2. If you are going to a new vet, have a place for your old vet's contact information so the new vet can call, or have them fax records or information that you did not bring with you.&lt;/P&gt;&lt;P&gt;3. You may want to break the check list up into two columns- it can become a little overwhelming when it's just one like ALL THE WAY DOWN!
