A long time ago I came across a great form to provide to your vet.  I used it on the old Cat Fancy board.  Too many times we are taking sick cats to the vet and we are upset and we can forget to provide important details to the vet.  I would GREATLY APPRECIATE if everyone would provide some feedback so we could turn this form into a really good form for our Cat Channel members to use when they take sick animals to the vets.

NEUTERED/SPAYED AND WHEN? 

AREAS WHERE CAT IS KEPT (PARTICULARLY IF YOU TRAVEL WITH YOUR PET) On a ranch, on a farm, in a barm, swamp area

Is your cat strictly indoors or outdoors?

When was your cat's last vet visit and for what?

Has your cat been vaccinated for FIV and FeLV and when?

This form is great. I have taken my older cat, Midnight, to the vet after a cat fight and have been too upset to think. Filling out most of the form before it is needed would be a good idea.

Beahaven,

Yes the form probably should be filled out in advance. How is Midnight doing now after his cat fight? Is he ok? Please let us know.

I think that this is a great form especially if you are travelling with your pet and you need to take your cat to a vet other than your own.

I think you should add "Past and Current Medical Conditions with dates".  A lot of times people have to take their cat to the vet and are so concerned and focused on the issue at hand they forget some of this important information.

I think this form is very helpful, the only thing to add is if the cat has any history of seizures or painfull area's to touch or any weird behavior. I think this form is a great idea and very important! thanks for thinking about it.

Hi, i am a vet tech studient from national american university in RC, SD. I am also a proud mommy of a beautiful, long haird tuxedo cat named sage. She is a spayed and declawed female with history of abuse(inflicted apon her) and at times she is quite anti social and aggressive. But i love her cause she has a survivor spirit that inspires me as well. I am looking forward to sharing stories and tips with everyone in this community.

Thanks Ragdoll Mom!!!

I used this form when I took Tina to the vet a few months ago for her first visit since her adoption.  My vet was impressed and put it in her file!
